We are looking for an outstanding Community Relations Representative (Sales) to join our team in Southeast Houston in anticipation of Unity’s expansion into the region.

Core Responsibilities:
- Develop and maintain relationships with designated referral partners
- Gain an understanding of your referral partners’ individual care needs and overall account management of assigned territory
- Be the point person to liaise between referral partners and the Hospice team
- Increase awareness, access and utilization of Unity's services of hospice & palliative care in our territory
- Build a large network and client base with the help of our hospice sales managers including 1:1 on-going field training to help you be successful in your referral goals and commission
- Participate in anon-call rotation for evening and weekend days
Day to Day Activities:
- Each day you will be visiting different physician offices, nursing homes, clinics, hospitals, and assisted living facilities to determine what our referral partners’ needs are
- Manage your own schedule to maximize in-person visits to accounts
- Communicate with referral partners and identify their needs via phone or email
- Work with hospice care team to ensure care meets expectations and preferences of referral partner
- Daily notes summarizing sales progression in our company CRM
- Meet with patients and families to identify their care needs and communicate how Hospice can be of benefit

- Hospice Experience Required
- Driver’s license, insurance, and vehicle
